Augment app upload issues

I'm trying to upload to the Augment app for the first time.  

 

I have a model and when I upload it says my model is empty:  "There are no vertices in the meshes"

 

Do I need to do something specific in FormZ to get it converted to .obj or .dae correctly?  I have tried both with no success.  The model looks like this:

If I upload .dae it says: Collada: Expected different index count.  Check that you exported with Collada's 1.4 version, or try to export in different supported format like OBJ.

it looks like you are using a subD model... i wonder if that's the issue?  try converting to facetted?

I tried converting to facetted prior to export, using Convert tool.  I also tried simply uploading a basic model of a cube.  It also says "There are no vertices in the meshes"
